On June 29, 2014, I received an e-mail from Rick Charles Herrick telling me that George Toriello had given him the form and information about our website. 

On July 3, 2014, I received the form in the mail. 

Rick Charles Herrick stated that he had graduated Hackensack High School in 1964 and then served in the Air Force from August of 1964 to September of 1968. He rose to the rank of AIC.  Rick wrote that his medals were of time and grade.
